ETONNE CURATED EYE
Transconjunctival Lower Blepharoplasty
Also called Under-eye Fat Repositioning, Transconjunctival Fat Repositioning
Under-eye fat repositioning (transconjunctival fat repositioning) is a surgery that removes and repositions bulging under-eye fat through the conjunctiva on the inner side of the lower eyelid. Unlike lower blepharoplasty, which also removes loose under-eye skin, it approaches and treats the fat from the conjunctival side. At ETONNE in Gangnam, Seoul (Gangnam-daero, Nonhyeon Station), the surgery is performed by board-certified plastic surgeons Dr. Cho Jeongmok, Dr. Kim Harkyoung, Dr. Kim Jaegon, and Dr. Koo Heetae.

Bright and Radiant Eyes
Transconjunctival Lower Blepharoplasty is a surgical technique aimed at improving the appearance of bulging fat under the eyes,
which can make one look tired and give a darker impression. This procedure involves removing and repositioning the fat through the conjunctiva
to eliminate under-eye puffiness, which can give a more youthful look.

POINT 1.
Detailed Diagnosis
Taking every natural feature of the face into account,
the eyes must be shaped to harmonize best with your own face.
ETONNE does not look at the eyes alone.
To determine what is needed for the most beautiful eyes,
we weigh and judge every condition of the patient's face.
1. Brow position and shape
2. Eye opening strength(blepharoptosis) and skin sagging(pseudoptosis)
3. Shape of the outer corner and horizontal length
4. Use of the eyebrows for eye opening (brow compensation)
5. Amount of redundant skin
6. Thickness of eyelid skin
7. Shape of the lower lid and beyond
CASE & SOLUTION
When brow to lid length is narrow
Brow position correction AND / OR Blepharoplasty
Puffy eyes
Brow position correction AND / OR Fat removal
When use of brow elevation during eye opening
Blepharoptosis correction AND / OR Blepharoplasty AND / OR Brow position correction
Eyes appear tired
Blepharoptosis correction
Corner of the eyes looks droopy
Lateral canthoplasty
Simple cosmetic issues without complicated problems
Blepharoplasty
Corner of the eyes look upslanted
1. Correction of inner corner or Brow position correction 2. Lateral canthoplasty and vertical lengthening 3. Combination of the above
Rather than just creating a line or enlarging the eyes,
the approach involves a comprehensive diagnosis considering various factors such as the distance between the outer corner of the eye and the orbital bone, eyebrow position, eyelid thickness, and the overall facial proportions for each case. This leads to a customized surgical plan and outcome.
By analyzing the area around each individual's eyes and proceeding only with necessary procedures, a natural and balanced eye shape is achieved, avoiding any exaggerated results.

Adding Detail to Naturalness
CURATED EYE SURGERY
Precautions
Swelling and bruising may occur after surgery, and how long they take to subside varies from person to person. In rare cases bleeding, infection, inflammation, or left–right asymmetry may occur, which may require additional treatment. Because the fat is treated through the conjunctiva (the inner surface of the eyelid), the whites of the eyes may swell or the eyes may feel as if something is in them in the early period after surgery; the under-eye area may look hollow or its surface may look uneven; and because loose skin is not removed, other methods are discussed as well if the skin sags significantly. Please tell us at your pre-surgery consultation about any medications you take, including blood thinners such as anticoagulants, as well as any medical conditions and whether you smoke. During recovery, please avoid alcohol and smoking and attend follow-up visits as scheduled. Please make your decision after you have been fully informed of the risks and of alternative options. Results and recovery vary from person to person.
Who It's For
When bulging fat under the eyes makes you look tired
When puffiness under the eyes makes your appearance look dull
When bulging under-eye fat, rather than sagging under-eye skin, is your main concern
Is this surgery for eye bags — bulging under-eye fat that makes me look tired?
Yes. Under-eye fat repositioning is a surgery to improve the tired, darker-looking appearance caused by bulging fat under the eyes. The bulging fat is removed and repositioned through the conjunctiva (the inner surface of the eyelid), and at ETONNE we determine whether it suits you after examining the amount of fat and the condition of the under-eye skin.
How is under-eye fat repositioning different from lower blepharoplasty?
Under-eye fat repositioning removes and repositions fat through the conjunctiva (the inner surface of the eyelid), while lower blepharoplasty also removes loose under-eye skin. At ETONNE, we choose the method after examining the amount of under-eye fat and the degree of skin sagging, and lower blepharoplasty is explained separately on the Lower Blepharoplasty page in the Anti-aging section.
Will there be scars?
Under-eye fat repositioning involves an incision in the conjunctiva (the inner surface of the eyelid), so no incision line is made on the outer skin. If the thread that holds the repositioned fat in place comes out through the skin, that thread is removed, and the marks where the thread passed and the course of recovery vary from person to person.
What are the anesthesia, surgery time, and recovery schedule?
The surgery takes about 45 minutes under local anesthesia or sleep anesthesia, and you go home the same day without hospitalization. If the thread holding the fat in place comes out through the skin, it is removed after about 3 days, daily activities are possible from about 4–5 days after surgery, and one follow-up visit is needed after surgery, though in some cases none is needed. How long swelling and bruising take to subside varies from person to person.
