Square jaw surgery is a surgery that resects the angular part at the back of the lower jawbone (the mandibular angle) where it looks wide or prominent, refining the jawline to look smooth and slender. The width and length of the jaw are considered together and only as much as needed is resected in a long curve, with the amount decided by considering the position of the nerve that runs through the lower jawbone and the condition of the soft tissue. Precautions
Swelling and bruising may occur after surgery, and how long they take to subside varies from person to person. A scar remains at the incision site, and in rare cases bleeding, infection, inflammation, or left–right asymmetry may occur, which may require additional treatment. Because this surgery resects the lower jawbone, opening your mouth or chewing may be uncomfortable in the early period after surgery. Because the surgery is done near the nerve that runs through the lower jawbone, the sensation around the lower lip and chin may become dull, and in rare cases, a nerve may be injured and reduced sensation may last a long time. If more bone than necessary is resected, the overlying flesh and muscle may appear to sag. Please tell us at your pre-surgery consultation about any medications you take, including blood thinners such as anticoagulants, as well as any medical conditions and whether you smoke. During recovery, please avoid alcohol and smoking and attend follow-up visits as scheduled. Please make your decision after you have been fully informed of the risks and of alternative options. Results and recovery vary from person to person.

A jaw can look square for various reasons, not only the lower jawbone but also soft tissues such as the chewing muscles, salivary glands, and fat, so the method differs by cause. When the muscle is well developed, Botox can reduce the muscle volume, but the effect diminishes after a certain period; when a wide, angular lower jawbone is the main cause, square jaw surgery, which resects the bone, is considered. At ETONNE, we identify the cause through examination and tests and then decide on the method.
Can square jaw surgery be done if the nerve line is low?
Before surgery, an X-ray is taken to check the position of the nerve that runs through the lower jawbone (the nerve line). Bone cannot be resected beyond the nerve line, so if the nerve line is low, there is a limit to how much can be reduced; in this case, the plan adjusts the amount and extent of cortical osteotomy, which works on the outer layer of the bone.
Can the flesh around the jaw sag after surgery?
If more bone than necessary is resected, soft tissues such as flesh and muscle lose their place to settle and may appear to sag. At ETONNE, our principle is to resect only what is needed, and we decide the amount by also looking at skin elasticity and the condition of the soft tissue; if sagging is a concern, we discuss methods such as lifting with you at the consultation.
Where will the scar be?
A scar remains at the incision site. The incision is made inside the mouth and closed with dissolvable sutures, so there are no stitches to remove. How the scar settles varies from person to person.
