Fat grafting is a surgery that injects autologous fat harvested from the abdomen or thighs into sunken areas of the face, such as the forehead, cheeks, temples, under the eyes, and the nasolabial folds, to restore volume. After the areas and amounts to graft are designed by looking at the balance of the whole face, fat harvested with a dedicated syringe and purified is injected in divided portions, layer by layer. Precautions
Swelling and bruising may occur after surgery, and how long they take to subside varies from person to person. Small scars may remain at the small openings through which fat is harvested, and in rare cases bleeding, infection, inflammation, or left–right asymmetry may occur, which may require additional treatment. Part of the injected fat is absorbed, and how much takes varies from person to person depending on constitution and the area, so if the volume decreases more than expected or a left–right difference appears, additional grafting may be needed. The injected fat may clump so that it can be felt, or the surface may look bumpy, and fat necrosis, in which part of the fatty tissue becomes a firm lump, may occur. The area where fat was harvested may also have swelling, bruising, or soreness, and its surface may look uneven or dented. If you have had filler injections before, please tell us the type of filler at your consultation. Please tell us at your pre-surgery consultation about any medications you take, including blood thinners such as anticoagulants, as well as any medical conditions and whether you smoke. During recovery, please avoid alcohol and smoking and attend follow-up visits as scheduled. Please make your decision after you have been fully informed of the risks and of alternative options. Results and recovery vary from person to person.

Autologous fat is injected into areas lacking volume, such as sunken areas of the face including the forehead, cheeks, temples, under the eyes, and the nasolabial folds. At ETONNE, we look at the balance of the whole face to decide the areas and amounts to graft, and when the forehead is severely sunken, we may recommend correcting it with forehead surgery rather than fat grafting.
Where is the fat harvested from, and will there be a scar?
Fat is harvested from the abdomen or thighs with a dedicated syringe through a small opening, so a small scar may remain there; the harvest site is stitched, and the stitches are removed after about 5–7 days. In the face, the fat is injected with a needle, so no incision is made. How the scar settles varies depending on your skin condition.
Does all the grafted fat stay? Is one session enough?
Part of the injected fat is absorbed and part takes hold and remains, and how much takes varies from person to person depending on constitution and the grafted area. At ETONNE, we aim to complete the treatment with a single graft, we do not graft frozen fat, and PRP or stem cell therapy may also be combined. However, if not enough fat takes, additional grafting may be needed.
Can fat grafting be done if I have had filler before?
Please check the type of filler you received before and tell us at your consultation. If you have had hyaluronic acid filler or the filler can be felt, the filler may be dissolved before fat grafting is done.
What are the anesthesia, surgery time, and recovery schedule?
The surgery takes about 30–60 minutes under local anesthesia or sleep anesthesia, and you go home the same day without hospitalization. Stitches are not removed from the face and are removed from the body after about 5–7 days, face washing and makeup are possible from the day after surgery, and about one follow-up visit is needed after surgery. How long swelling and bruising take to subside varies from person to person.
